Our Office Policies at Elevate Dental Wellness

Our Financial Policy

For your convenience, Elevate Dental Wellness offers a range of options to make your payment as easy as possible. We accept the following:

  • Cash
  • Personal Checks
  • Major credit cards including Visa, MasterCard, Discover, American Express
  • Financing through CareCredit and LendingClub

We’re here to help you understand and maximize your benefits from your insurance plan. We’ll gladly file your insurance electronically as a courtesy to you, and you’ll be directly reimbursed by your insurance company. Please note that we’re not preferred providers for any insurance plans.

Full payment is due at the time services are performed. Please do not hesitate to ask us any questions about our financial policy. Give us a call at (970) 279-5647 to speak to a knowledgeable member of our staff.

Rescheduling Policy

We reserve your appointment time especially for you, so we require a 24-hour advance notice if you’re unable to keep your scheduled appointment. If you provide less than 24 hours notice, your account will be charged a $50 late cancellation fee.

If you need to reschedule an appointment, do so by calling during normal office hours. Please don’t leave a message on the practice voicemail.

Treatment of Minor Children

The parent who accompanies a minor for treatment is responsible for any payments for services rendered at the time of the appointment, regardless of who is the primary insurance policyholder.

Will my dental insurance cover the cost of treatment?

Most dental insurance companies will cover a portion of your treatment, but not all of it. That’s why we suggest contacting your insurance provider before your appointment to get an understanding of your coverage. For your convenience, we can file your insurance electronically for you and help make sure you receive the maximum benefits possible.

Can my child receive care at Elevate Dental Wellness?

Yes! Our holistic-centered practice offers safe, all-inclusive care for patients of all ages, both young and old. We use natural dental products that contain raw ingredients, which are safe for the entire family.
